Cellulite is one of the top cosmetic concerns among women. Several treatments exist for cellulite, but few offer a long lasting solution. This is partially because cellulite forms for many reasons and throughout our lifetimes. Therefore, it’s not possible to get rid of cellulite forever because it is influenced by natural factors such as weight changes and age that are not possible to prevent. The FDA recently approved a game changing new cellulite treatment that has shown promise in providing long lasting results. The treatment is known as Qwo. Cellulite is a harmless skin condition that causes dimpling on the surface of the skin. It affects approximately 90% of women and many compare cellulite to the lumpy look of cottage cheese. Cellulite most frequently occurs in the naturally fatty areas of the body such as the buttocks, thighs, lower abdomen, and hips.

Cellulite forms when fat cells expand and push upward against the skin. This is not always associated with weight gain and sometimes happens due to hormone changes. Within the anatomy of the buttocks, there are fibrous bands of collagen known as fibrous septae. These connective tissues lay between collections of fat cells. They connect the skin to other structures such as the muscles and other tissue. When fat cells shift and push against the skin, this causes the bands to tense and pull downward. Thus, this creates the recognizable dimpling of cellulite.

Many components contribute to the formation of cellulite. While weight and lack of exercise are often touted as the main culprits behind cellulite, this is not entirely true. Cellulite is genetic and occurs in women of all ages, sizes, and lifestyles. Lifestyle and weight fluctuations can certainly contribute to the severity of cellulite and in some cases cause it all together, but they are not the only factor that influences cellulite production. 

Grades Of Cellulite

Cellulite ranges in severity from mild to severe. This is broken down into three different grades of cellulite. As the grade increases, treatment becomes more difficult and patients with severe dimpling may require combined treatments or more sessions. Though, cellulite at these grades is not necessarily untreatable. The grades of cellulite are as follows:

  • Grade 0: Not visible in most circumstances, including when pinched or standing. However, structural changes consistent with cellulite occur beneath the skin.
  • Grade 1: Dimples may not be immediately noticeable. The skin may look paler and thinner. When pinched an “orange peel” effect may occur, where the surface of the skin resembles the texture of an orange peel.
  • Grade 2: Dimples and lumps become more prominent. They are likely most noticeable when standing up and may even completely disappear when laying down.
  • Grade 3: Cellulite visible at all times and in all positions. This constitutes the most severe cellulite and dimpling.

    Qwo is an injectable enzyme known as collagenase clostridium histolyticum. This enzyme breaks down collagen. When injected into the dimpling areas, it targets the fibrous septae bands and releases the tension that causes cellulite. Since it treats the anatomical structure of cellulite, Qwo can treat cellulite with any cause.

    In summer 2020, the FDA approved the use of Qwo for the treatment of cellulite in the buttocks of adult women. Clinical trials and studies commenced for seven years–the longest of any other cellulite treatment–and continue today. These studies have indicated that Qwo may prove to be the most effective treatment for cellulite.

    Qwo is a game changer for treating cellulite. Before Qwo’s release, the most effective cellulite treatments were painful or surgical. Other treatments like cellulite creams or at-home remedies varied in effectiveness between patients. They also did not adequately treat moderate or severe cellulite.


    When you undergo Qwo injections at Seattle Plastic Surgery, your provider will take you to one of our private treatment rooms. You will undress and lay on your stomach. Your provider may offer numbing cream, though most patients find it unnecessary. Generally, your provider will treat only one or two small sections per treatment. Each section may consist of a couple of deep dimples or a few centimeters of shallower dimpling. 

    After preparing the injections, your provider will inject the cellulite. Injections begin at a perpendicular angle. Without fully removing the needle, they will shift the angle forward and then backward. They will repeat this process three to six more times depending on your goals and specific treatment needs. 

    Qwo injections can take as little as ten to fifteen minutes. You can then return to your daily activities immediately following treatment. You will then return two more times with each appointment spaced roughly 21 days apart. Once you finish this initial round of treatment, you and your provider will assess your results about a month later. At this point, you and your provider can determine whether you need further sessions or not.

    Expected Results

    Many patients begin seeing results after their second appointment. Though, this will depend on many factors such as the severity of dimpling and your body’s response to treatment. Following a complete three session treatment, patients usually start to see the appearance of cellulite reduce for smoother buttocks. 

    If you have more widespread dimpling, results will be localized to the treatment area. Subsequent Qwo injections can provide results in other areas. Your provider will give you a more detailed explanation of the results you can expect based on your anatomy and goals.


    Why Is Cellulite More Common In Women?

    There are two main reasons why cellulite is more prevalent in women than men. First off, cellulite production is associated with estrogen. Since women have higher levels of estrogen, this contributes to why cellulite is more common among them. Additionally, when estrogen levels increase or drop, such as during puberty, pregnancy, and menopause. During these times, women are more likely to see the development of or changes in cellulite. 

    The second reason women are more prone to cellulite has to do with the fibrous connective tissue mentioned earlier. In women, this is arranged vertically, which allows fat to press against the skin. However, in men, this tissue is in a criss-cross pattern which prevents cellulite from forming.

    This does not mean that men are immune to cellulite. Yet, they are much less likely to see cellulite form, even with weight changes. Certain hormonal changes or other factors can cause cellulite in men.

    Do The Injections Hurt?

    There may be some minor discomfort during the injections. However, patients rarely find the need for numbing agents and many say it is nearly painless. Seattle Plastic Surgery does offer topical numbing cream when necessary.

    Is Qwo Safe?

    Qwo is perfectly safe for cellulite reduction. There are some slight risks associated with collagenase injections such as infection. However, the risks are no more than those of other injection procedures. It is possible to be allergic to collagenase, though it is rare. If you know you have a collagenase allergy, your provider will likely recommend other types of cellulite treatment.

    Does Qwo Have Side Effects?

    Qwo injections can cause some minor and temporary side effects. Not every patient will experience side effects. Patients that do get side effects may see swelling, bruising, redness, and itchiness around the injection sites. All side effects usually go away within one to two weeks. Additionally, after the first session, side effects at subsequent treatments typically are less severe or may cease.

    Can Qwo Treat All Areas Of Cellulite?

    Early studies have shown that Qwo likely can reduce cellulite in all the common areas it collects. However, the FDA has only approved Qwo for use in the buttocks. Qwo will likely receive future approvals for other areas of the body. For now, Qwo only treats cellulite in the buttocks.
